Our Mission & Purpose

Established in 2022, Queen City Community Connect Coalition unifies CHWs across organizations in a collaborative forum by creating an environment that equips and empowers those to serve Mecklenburg County communities.

Our mission is to unify, equip, and empower CHWs to serve Mecklenburg County communities. 

Our Objectives

Convene CHWs in a collaborative forum that nurtures workforce and professional development opportunities.
Increase public and professional recognition of CHW expertise and contribution.
Equip CHWs with relevant resources by sharing active health and social services that serve Mecklenburg County communities.

Roles of
Community Health Workers

According to the CHW Core Census (C3) Project, CHWs have 10 core roles. 

Cultural Mediation

CHWs educate individuals and communities about how to use health and social services. Additionally, they educate systems about cultural norms.

System Navigation

CHWs participate in care coordination and/or case management, making referrals and providing follow-up to inform people and systems.

Advocacy

CHWs conduct policy advocacy, advocating for communities’ basic needs, resources, and perspectives.

Implementing Assessments

CHWs participate in the design, implementation, and interpretation of individual and community-level assessments.

Conduct Outreach

CHWs recruit and engage in case-finding individuals, families, and community groups to services and systems.

Cultural Competency

CHWs conduct health promotion and disease prevention education specific to the cultural needs of communities while providing necessary information.

Coaching and Social Support.

CHWs provide individual support and coaching, encouraging residents to obtain care and other services to support disease prevention.

Building Capacity

CHWs build individual and community capacity through training, support, and education with peers and among CHW groups.

Providing Direct Service

CHWs provide basic screening tests and services such as taking vital signs, diabetic foot checks, and connect residents to resources.

Participate in Research

CHWs engage in evaluating services and programs, as well as participate in identifying and engaging residents in research and community consent.
