SUMMARY:Addressing SDoH to Improve Population Health (CHWs N' Action Webinar Series)
DESCRIPTION:Description:\n\n\n\nSocial determinants of health (SDoH) are the conditions in the environments where people are born\, live\, learn\, work\, play\, worship\, and age that affect a wide range of health\, functioning\, and quality-of-life outcomes and risks. They include things like economic status\, education\, healthcare access\, neighborhood quality\, and social support. Evidence is building that these non-medical factors have a significant impact on people’s health. What is the community health worker (CHW)’s role in improving these “upstream” health factors? We will discuss how CHWs influence quality of life issues as defined by the 2022 Mecklenburg Health Opinion Survey (see pages 32-36 of 2022 Mecklenburg County CHA). We will also highlight organizations and programs in Mecklenburg County that are impacting SDoH. \n\n\n\n\n\n\n\n\n\nObjectives:\n\n\n\n\nDefine SDoH and how it impacts individual\, community\, and population health.\n\n\n\nDescribe how CHWs can address SDoH in Mecklenburg County.\n\n\n\nDiscuss how CHWs work across organizations and professions to connect individuals to programs and services  \n\n\n\nExamine additional ways that organizations in Mecklenburg County can collaborate with CHWs to continue improving upstream health factors.\n\n\n\n\n\n\n\n\n\n\nTarget Audience:\n\n\n\nNovice and Seasoned CHWs\, Practice Administrators\, Public Health Practitioners\, CHW Supervisors\, Health and Social Care Team Members\, Public Health Team Members\, Community and Clinically Integrated Networks\, and others interested in and/or engaged in improving population health. \n\n\n\n\n\n\n\n\n\nREGISTER\n\n\n\n\n\n\n\n\nTHANK YOU TO OUR COMMUNITY PARTNERS
