CHW N' Action

Explore our three-part series in partnership with Mecklenburg County Public Health Department, South Piedmont AHEC, and the Center for Community Health Alignment. Each webinar features a presentation by experts in the topic of interest and a panel discussion introducing 1-2 CHWs and/or Promotores in the field making an impact in their community.

Community Health Workers 101

The first module serves as a foundational webinar reviewing the definition, roles, competencies, and settings for CHWs. This orientation will explore why CHWs are an expanding profession and review some of the benefits of their work.  

Objectives:

  1. Define CHWs as a profession
  2. Identify CHW roles and competencies according to the CHW Core Consensus (C3) Project
  3. Describe the benefits of adding a CHW to a health and/or social care team
  4. Distinguish how clinical and community CHWs may operate differently in practice and in scope
  5. Identify how clinical and community settings can collaborate with CHWs

Advancing Health and Racial Equity

The second module takes a deeper dive in understanding how CHWs serve in their communities to help transform health disparities through their innovative approach to engage community. Topics covered will help CHWs advance health and racial equity in the workplace, communities, and within the CHW profession.

Objectives:

    1. Define health equity.
    2. Explain how structural racism contributes to health and race inequities.
    3. Describe how CHWs work to achieve health and race equity.
    4. Discuss what health and race equity means for community and population health.

Addressing Social Determinants of Health to Improve Population Health

The third module concludes on how CHWs can work upstream to address social determinants of health by positivity impacting qualify of life issues as defined by the 2022 Mecklenburg Health Opinion Survey. Discussions highlighted how organizations and programs in Mecklenburg County are impacting social determinants of health.

Objectives:

    1. Define SDoH and how it impacts individual, community, and population health.
    2. Describe how CHWs can address SDoH in Mecklenburg County.
    3. Discuss how CHWs work across organizations and professions to connect individuals to programs and services  
    4. Examine additional ways that organizations in Mecklenburg County can collaborate with CHWs to continue improving upstream health factors.
