Established in 2022, Queen City Community Connect Coalition unifies CHWs across organizations in a collaborative forum by creating an environment that equips and empowers those to serve Mecklenburg County communities.
Our mission is to unify, equip, and empower CHWs to serve Mecklenburg County communities.
According to the CHW Core Census (C3) Project, CHWs have 10 core roles.
CHWs educate individuals and communities about how to use health and social services. Additionally, they educate systems about cultural norms.
CHWs participate in care coordination and/or case management, making referrals and providing follow-up to inform people and systems.
CHWs conduct policy advocacy, advocating for communities' basic needs, resources, and perspectives.
CHWs participate in the design, implementation, and interpretation of individual and community-level assessments.
CHWs recruit and engage in case-finding individuals, families, and community groups to services and systems.
CHWs conduct health promotion and disease prevention education specific to the cultural needs of communities while providing necessary information.
CHWs provide individual support and coaching, encouraging residents to obtain care and other services to support disease prevention.
CHWs build individual and community capacity through training, support, and education with peers and among CHW groups.
CHWs provide basic screening tests and services such as taking vital signs, diabetic foot checks, and connect residents to resources.
CHWs engage in evaluating services and programs, as well as participate in identifying and engaging residents in research and community consent.
